It arises from deliberate, even obsessive, reflection on failure and a constant searching for new solutions.</p></blockquote>
<p>Exactly.</p>
<p>Even though Gawande writes about medical issues, he does so in a way that imparts a lot of wisdom that can be applied to many endeavours, including innovating.</p>
